請遵守下列規定以及作業時間以利門禁權限開通，如有任何問題請聯繫湯小姐(#7154)。
注意事項:
- 所有已通過[動物中心使用說明會]者皆可申請門禁權限
- 需使用(含)2020年後的[動物中心使用說明會]出席證明申請權限
- 單次開通以2年為限，2年後需再次參與[動物中心使用說明會]並通過考試，通過後請聯絡承辦人做延續開通作業
- 可拿欲開通權限之感應晶片卡感應門禁機，如門禁機有反應，即該卡可設定開通權限
- 如需開通特殊門禁(3F行為學儀器室)，請通過行為學儀器操作考試後聯絡門禁承辦人開通權限
- 舊制門禁系統感應扣於2022/12/31前歸還，逾期不受理後續作業(2023/01/01起不受理任何作業)
註:每月月底表單將會關閉，日期更新完成後再開啟

Please abide by the following regulations and operating hours to facilitate the opening of access control. If you have any questions, please contact Ms. Tang (#7154).
1. All those who had passed the [Introductory seminar of LAC] can apply for access control
2. It is necessary to pass the test of [Introductory seminar of LAC] which was hold after(include) 2020
3. After 2 years, you need to participate in the seminar again and pass the test. After passing the test, please contact the undertaker to continue the opening operation.
4.Please fill in the online form to make an appointment for access control opening time:
